Duties include but are not limited to:
- Providing comprehensive medication management to patients with mental health conditions. The includes but is not limited to initiating, modifying, or discontinuing medication therapy, ordering labs and other tests to help determine efficacy or toxicity of medications, designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence or VHA and/or published guidelines
- Reviewing patient medication regimens for clinical effectiveness, medication selection, dosing, contraindications, side effects, and potential medication interactions, and therapeutic outcomes as required
- Reviewing inpatient psychiatric charts for medication safety, appropriateness, and effectiveness. Collaboratively rounds with interdisciplinary inpatient treatment team and makes clinical recommendations as indicated
- Documenting clinical interventions and therapeutic plans accurately in the electronic medical record in a timely and professional manner.
- Collaborating with BHIP team to provide pharmaceutical care to patients with mental health conditions
- Reviewing and evaluates requests for prior authorization and restricted medications for appropriateness and compliance with established criteria where applicable
- Providing responses to medication information inquiries from all customers
- Documenting interventions using workload capture tools as instructed (e.g. PhARMD tool)

Work Schedule: 8:00 am - 4:30 pm, Monday through Friday
Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
