Duties:
- Responsible for developing, interpreting, and administering medical services for Veterans enrolled in the HBPC Program.
- Assesses the health status of Veterans through interviews, health histories, home visits, physical examinations, and diagnostic testing.
- Evaluates the physical and psychosocial health status of individuals and groups.
- Promotes health maintenance, wellness, and illness prevention.
- Manages patient care within the scope of professional preparation, including prescribing medications when appropriate.
- Implements care plans through independent clinical decision-making, health counseling, and collaboration with the HBPC team.
- Focuses on maintaining optimal health, preventing illness and injury, and managing selected acute or chronic health conditions.
- This position requires traveling from Lyons or East Orange to the Veterans Home, depending on the needs of the service.
Work Schedule: Monday - Friday 8:00 am - 4:30pm
