The PACT Physician is responsible for providing comprehensive primary care services to a defined panel of patients at the Veterans Health Administration. This position is integral to the PACT model, where the physician works collaboratively with a team of healthcare professionals to ensure coordinated, patient-centered care

Duties and Responsibilities:

- Provide direct patient care to a panel of adult patients, managing both acute and chronic conditions.

- Educate patients and families regarding health maintenance, disease prevention, and treatment protocols.
- Participate as an active member of the PACT, working closely with nurses, administrative staff, social workers, pharmacists, and dietitians.

- Maintain complete and accurate medical records in compliance with VHA policies, regulations, and standards.
- Participate in quality improvement initiatives within the clinic and service line.
- Conduct history and physical examinations, order and interpret diagnostic tests, and develop treatment plan
- Administer preventive care procedures and manage comprehensive care plans for patients with multiple chronic conditions.
