Duties include but not limited to:
- Provides primary care for a panel of patients using the medical home model of care, as part of an interdisciplinary member of the team.
- Practices, promotes and implements current safety policies and procedures.
- Assesses safety needs of patients and recognizes opportunities to improve environmental and operational safety.
- Identifies quality improvement (QI) issues, participates in QI monitoring and evaluates clinical practice.
- Utilizes QI findings to improve quality of care.
- Provides leadership in adhering to and promoting practice based on current standards of clinical practice.
