The Geriatrics/Long Term Care (G/LTC) Service, Community Living Center (CLC) Physician provides inpatient care to CLC Residents. This includes after hours telephone coverage for Veterans admitted to the CLC Houses.
In addition to the general duties described above, specific duties include the following functions, but are not limited to:
- Provides clinical and oversight for all Resident treatment plans in collaboration with a case manager or interdisciplinary treatment team.
- Participates as part of the Interdisciplinary team to plan the care of the Resident, including resident-specific goals and outcomes for discharge. Provides clinical input and demonstrates leadership as a member of the interdisciplinary team. Participates in Interdisciplinary team meetings.
- Provides education to the G/LTC team and Veteran Affairs Medical Center staff on current geriatric information and clinical practices.
- Completes appropriate documentation as administratively required such as Minimum Data Set (MDS), referrals, encounters, etc.
- Completes orders and follows up on results, including lab, radiology, consults, medications, diets, code status and other needs to treat the Resident.
- Completes routine rounds and evaluations as needed.
- Mentors rotating medical students, residents and other trainees during G/LTC rotations.
- Works directly with staff to identify needs, system issues, and communicate goals for the CLC.
- Will be a leader in performance improvement. Participates in quantitative and qualitative data review to support performance improvement/quality assurance efforts of the CLC.
- Provides patient education in an interactive setting.
- Serves on local and VISN committees to represent CLC concerns and initiatives.
- Willing to work weekends and be on call for coverage in the CLC. Supports the 24/7 coverage system for CLC Residents.
- Other duties as assigned.

Pay: Competitive salary, annual performance bonus, regular salary increases
