The Geriatrics & Extended Care Physician will provide high quality patient centered care to Veterans assigned at the Community Living Center, Home Based Primary Care, Geriatric Evaluation Management (GEM) clinic and Palliative/Hospice Care Services at the Coatesville VA Medical Center, Pennsylvania
.
Duties include but are not limited to:
- Performs assessment, diagnosis, treatment and outcome measures of healthcare issues, histories, and physicals, ordering diagnostic and therapeutic tests/studies, initiating consultations and referrals, and prescribing medications/supplies for patient at home.
- Communicates and consults regularly with other providers in primary and specialty care to assist in the care of geriatric patients as needed.
- Performs evidence-based practices to enhance symptom management of geriatric patients to include Palliative and end of life care when appropriate.
- Consults with the interdisciplinary care teams to establish a written plan of care.
- Provides clinical oversight if applicable to Physician Assistants and Nurse practitioners who function within these services.
- Provides inpatient coverage in CLCs (Community Living Centers), Medical officer of the day, Palliative/Hospice care.
- Participating in pre-admission screening, discharge planning, and interdisciplinary team rounds.
Work Schedule: Monday - Friday, 8:00 am - 4:30 pm
